CME Credit Information for a Regularly Scheduled Series (UMHS personnel)
Holding a CME activity for which credit is designated involves four stages:
- Application for credit designation
- Additional actions to address potential commercial influence (i.e., Disclosure)
- Reporting on the activity (final documentation)
Information relevant to each of these steps is presented below with links to download applicable materials.
Fees No separate fee is charged for designating credit for a regularly scheduled series planned for UMHS personnel. The UM Faculty Group Practice annually funds this cost.
Note: Effective July 1, 2014, applications needing a full review in ten business days or less will be charged a $200 rush fee.

2. Application for credit designation
Application for Category 1 CME Credit for a Regularly Scheduled Series This online form must be completed and submitted. It contains detailed instructions. The last page of the application has a checklist of additional items that are or may be required.
CME Disclosure Form (PDF). A completed form for each planner must accompany the application. For planners with relevant commercial relationships, the "Resolution" section at the form’s bottom must be completed.
3. Additional actions to address potential commercial influence
Disclosure of existence of relevant relationships with commercial companies (using CME Disclosure Form).
Review and resolution of any disclosed conflict of interest (bottom of CME Disclosure Form).
Disclose the relationships of both the planners (from #2 above) and presenters to the audience before the substantive educational presentations take place.
CME Disclosure Form (PDF) The Faculty Planner submits a copy of the completed form(s) for the CME file. For presenter’s with relevant commercial relationships, the “Resolution” section at the bottom must be completed.

DISCLOSURE TO PARTICIPANTS Listed below are examples of how to convey the disclosure information. For the CME file the Faculty Planner submits a copy of the disclosure summary and documentation that it was provided.
Annual Disclosure Summary – Example (PDF) (DOC) For many regularly scheduled series, a core group of individuals both attend and present to one another on a regular basis. In this case, each member of the "core group" completes a CME Disclosure form. The Faculty Planner compiles the information and distributes the summary for planners and presenters to the participants in the core group.
Email Disclosure – Example (PDF) This disclosure method is used for activities whose sessions are routinely announced to potential participants via email. The disclosure information for all Planners and Presenters (including disclosure of no relevant financial relationships) should be included in the text of the email message.
Disclosure via Handout – Example (PDF) This disclosure method is used for activities that distribute or display educational materials to participants at every session. The disclosure information for all Planners and Presenters (including disclosure of no relevant financial relationships) must be placed toward the beginning of the handout material, so that participants can view the disclosure prior to the start of the session.
Printed Disclosure – This disclosure method is used for activities that post information for participants to view at every session. The disclosure information for all Planners and Presenters (including disclosure of no relevant financial relationships) must be placed at the session entrace or at a registration table, so that participants can view the disclosure prior to the start of the session.
A written agreement must document that the support is not affecting the educational content.
The receipt of support must be acknowledged to the audience before the substantive educational program begins.
Written Agreement for In-Kind Commercial Support – Example (PDF) This or a similar form is to be completed for each instance of in-kind commercial support. The Faculty Planner submits a copy of the agreement for the CME file.
Printed Acknowledgement of In-Kind Commercial Support – Example (PDF) In-kind commercial support may be acknowledged in various formats, included in this example. For the CME file the Faculty Planner submits a copy of the acknowledgement and documentation that it was provided.
